CCP Parent Application

To be filled out by the parent. Families who are of Native American descent, who live on or near the FPST reservation/colony, may receive child care services for their children ages 0 to 12 years and up to age 18 for children with disabilities. Return back to the Child Care Coordinator Lynette Fisherman.

CCP Provider Application

To be filled out by the child care provider.

JOM Parental Assistance

Help with assistance to Federally Recognized Native American students, class/lab fees, school supplies, graduation gowns, correspondence fees, sports fees, uniforms and testing fees. Complete and return to Education Specialist, Chantelle Johnson.

Tribal Gym Rental Application

Rental application for Community Room, Kitchen and Gym located at the Education Department on 8955 Mission Road. Complete and turn into Tribal Administration.

After School Program Application

Our After School Program serves students in 1st -12th grade on Mondays-Thursdays. Students are bused from school to the Reservation’s Education Building  by Churchill County school buses and arrive around 3:35 pm. Homework assistance is provided, as well as opportunities to utilize the computer lab for individualized learning, the library to read, and additional activities such as culture, recreation, STEAM, prevention, arts and crafts, and character education. A snack is also provided. Students are transported home at the end of the day by After School Program staff. See under forms for an application.

The Education Department is also involved in working with Recreation to provide an enriching summer program.

Adult Vocational Training Application (certifications) Return to the Education Department.

Higher Ed Application

Full time college student 12 credits or more. Return to the Education Department.

Part-time college students. Return to the Education Department.
