POSITION DESCRIPTION

The Diabetes Prevention Coordinator will assess, educate and counsel patients with diabetes and pre-diabetes; participate as a member of the diabetes health care team; and track all diabetic and pre-diabetic patients, screen patients for diabetes, and ensure that diabetics are treated therapeutically according to the most modern, nationally accepted standard.

ESSENTIAL DUTIES & RESPONSIBILITIES

  1. Responsible for the direction, coordination, implementation, and evaluation of the diabetes prevention and education efforts on the Fallon Tribe.
  2. Review, evaluate, and monitor the Special Diabetes Program for Indians (SDPI) grant to ensure adherence to funding objectives including but not limited to outreach, registration, session implementation, curriculum development, wellness activities, data collection, patient education, and reporting.
  3. Maintain the Diabetes Registry to support meeting the SDPI outcome measures and objectives. Utilizes the registry to provide recommendations for population health strategies related to diabetes outcomes.
  4. Prepare and submit quarterly, semi-annual, and annual reports within specified time periods.
  5. Provides extensive information on diabetes prevention, care, and management to patients, staff, and tribal/community groups as requested.
  6. Periodically review the health status and needs of Native Americans by involving patients and families through surveys and/or focus groups, to identify health problems and develop educational opportunities, activities, and utilization of facilities.
  7. Advocate for the integration of chronic disease prevention and health promotion initiatives in the planning and development of activities and programs and in collaboration with other community agencies.
  8. Collaborate with other service providers to plan and coordinate educational activities.  Serves as liaison with local, state, regional, tribal, and federal/national agencies and organizations.
  9. Assists with coordinating wellness activities such as health fairs, run/walks, and tribal employee wellness program including providing structure, ordering supplies, promoting activity and recruiting participants.
  10. Coordinates healthy cooking demonstrations at wellness center, senior center, and other places of public interest to promote healthy eating habits and to promote diabetes prevention.
  11. Collaborate with local schools, educational, and youth/elder groups to implement wellness policies and provide Diabetes prevention education and activities.
  12. Communicate with patients and the tribal community on a consistent basis through a variety to methods such as newsletters, social media, e-mail, posters, etc. to provide information about diabetes prevention, healthy lifestyle changes, and program activities.
  13. Oversee the Toi Ticutta Wellness Center, associated equipment, and ensuring the center is operating in accordance with establish health and safety standards and ensure the building and equipment are clean, well maintained and secure.
  14. Make sure trainers adhere to safety guidelines by putting away any used equipment during and after training sessions; report all unsafe/maintenance issues immediately.
  15. Ensure new wellness center personnel are trained on safety, CPR/AED, etc.
  16. Provide direct supervision, coaching, and mentoring of assigned personnel.
  17. Other job-related duties as assigned.

KNOWLEDGE, SKILLS & ABILITIES

  1. THIS POSITION REQUIRES COMPLIANCE WITH THE INDIAN CHILD PROTECTION and FAMILY VIOLENCE PREVENTION ACT PUBLIC LAW 101-630; 25 CODE of FEDERAL REGULATIONS PART 63 – INDIAN CHILD PROTECTION and FAMILY VIOLENCE PREVENTION; CRIME CONTROL ACT OF 1990, EMPLOYEE BACKGROUND CHECKS; and EXECUTIVE ORDER 12968, ADJUDICATIVE GUIDELINES AND THEREFORE MUST SUCCESSFULLY COMPLETE AN EXTENSIVE BACKGROUND CHECK BEFORE START EMPLOYMENT.
  2. This position requires that the candidate be a mandated reporter.
  3. Ideal candidate would be a health care professional such as a CNA, MA, LPN, RN, or RD.
  4. Working knowledge of the prevention, diagnosis, and management of diabetes mellitus type 2.
  5. Excellent oral and written communication skills are required, including ability to make public presentations, and to work with local governments.
  6. Good knowledge of community resources related to medical care and public health.
  7. Must have knowledge of management principles and grant management.
  8. Knowledge of the HIPPA Law and the Privacy Act and be able to maintain confidentiality of records.
  9. Must be able to work with American Indian people on an individual and group basis.

QUALIFICATION REQUIREMENTS

  1. Associate’s Degree in a health-related field or two to three years’ experience managing a diabetes program in a tribal or community health center setting.
  2. Two (2) years prior experience in a supervisory or management position overseeing personnel in a health or related facility.
  3. Current BLS certification or able to obtain within first month of employment.
