This position involves providing personal care, housekeeping, ROM (Range of Motion) exercises, assisting with medications, and providing companionship to the elderly, disabled, and medically ill clients on the Fallon Reservation and Colony in maintaining and restoring home life.



  1. Provides personal care which may include: bathing, oral hygiene; grooming; changing bed linens; assisting with dressing and undressing; assisting with toileting activities; skin care to prevent breakdown; and keeping client’s living area clean and orderly, as appropriate
  2. Planning and preparing nutritious meals, following dietary requirements as instructed from care plan; assists in feeding the client, if necessary.
  3. Takes and records clients pulse, temperature, and respiration when required (with appropriate completed/demonstrated skills competency).
  4. Assists in ambulation and exercise according to care plan.
  5. Performs range of motion or other simple procedures as an extensional therapy service when required (with appropriate completed/demonstrated skills competency).
  6. Gives clients medication under nurses’ or doctors’ orders; assists clients in the self-administration of medication.
  7. Accompanies clients to doctors’ office and the hospital to provide assistance
  8. Performs light house cleaning and does laundry, as appropriate.
  9. Assists with applicable errands, such as shopping for food or household supplies.
  10. Meets safety needs of client’s and uses equipment safely and properly (i.e., foot stools, side rails, etc.).
  11. Reports on client’s condition and any significant changes to the Community Health Nurse.
  12. Provides instruction to clients, emotional support, and companionship.
  13. Encourage maintenance of family relationships, both nuclear and extended, and with friends
  14. Coordinate transportation through the Fallon Tribal Health Center, Stillwater Tribal Senior Center, and/or local transit services for patients/clients for local shopping and medical appointments.
  15. Adheres to the programs documentation and care procedures; maintains confidentiality and HIPAA compliance.
  16. Other job duties as assigned by the supervisor.



  1. High School diploma or equivalent required.
  2. Preference for Nurse Aide Certification and/or Work experience as home care aide or nurse’s aide.
  3. Have current Food Handlers card and SafeServ certificate (or obtain one within 30 days of hiring) and maintain such certification.
  4. Must have or be able to obtain CPR, First Aid, and AED or BLS certification within 3 months of hire and maintain such.
  5. Must be bondable.



  1. This position requires compliance with the Indian Child Protection and Family Violence Prevention Act (Public Law 101-630); Indian Child Protection and Family Violence Prevention regulations (25 CFR Part 63); Employee Background Checks (Crime Control Act of 1990); and Adjudicative Guidelines (Executive Order 12968) and therefore must successfully complete an extensive background check before the start of employment with the Tribe.
  2. Act as an approved Mandatory Reporter of suspected incidents of child/elder abuse or neglect.

  1. Knowledge of first aid, CPR, and other basis emergency medical care techniques and methods.
  2. Ability to demonstrate maturity of attitude toward work assignments; to converse effectively on the telephone; and to work under intermittent supervision
  3. Ability to deal with minor emergencies arising in connection with assignment.
  4. Ability to work under stress in a crisis situation
  5. Demonstrated ability to work with Native American people in culturally diverse environments; knowledge of/respect for Fallon Tribal traditions and values; heartfelt commitment to the health and welfare of Tribal and other elders.
  6. Maintain strict confidentiality and safeguard the privacy of elders.
  7. Potential hazards include exposure to chemicals, needles/syringes and infectious exposure; and occasional exposure to moving mechanical parts and medical equipment
  8. The employee is occasionally required to sit, stand, reach, lift, bend, kneel, stoop, climb, push and pull items. May need to help individuals up from sitting or reclining positions.
