POSITION DESCRIPTION

The CHR Supervisor assures that each CHR’s Scope of Work is in compliance with the guidelines established by the Fallon Tribal Health Clinic; serves as CHR when necessary, providing community health and in-home health services to the elderly, disabled and other eligible patients that cannot provide for themselves.  The CHR Program endeavors to provide quality outreach health care services and health promotion/disease prevention services to American Indians and Alaska Natives within the communities of on the Fallon Paiute-Shoshone Tribal Reservation and Colony.

ESSENTIAL DUTIES & RESPONSIBILITIES

  1. Maintain the confidentiality, security and integrity of the patient’s care and information.
  2. The CHR Supervisor is responsible for assuring that each CHR/Transporter submits program data as required.
  3. The CHR Supervisor assesses, updates, and evaluates the training needs of CHRs and assures that each CHR is properly trained consistent with the CHR’s contract and scope of work.
  4. CHR Supervisor facilitates coordination and effective interaction between Tribal Program CHRs.
  5. Provides close supervision of CHRs, which includes services performed, reporting requirements, and continuing education requirements.
  6. Endures effective provision of program services including in-home health care to patients as per treatment plan established and reviewed by Physician/Medical Director or Public Health Nurse, health education/counseling, case management and coordination, transportation and delivery.
  7. Assist clients who are applying for Social Security, Medicaid, and Medicare etc.
  8. Assist with arranging medical appointments and transportation.
  9. Meet with Public Health Nurse and clinic Physician on a routine basis  (at least monthly) to review client treatment plans.
  10. Prepare and submit a monthly narrative report to Associate Director of Business Services
  11. Responsible for submitting patient care complaints (PCC’s) into RPMS database.
  12. Document all care and treatment given on a daily basis.
  13. Prepare PCC and Super bill for medical record.
  14. Respond to all patient inquiries and refer to appropriate resource if needed.
  15. Serve as liaison between the people in the community, clinic staff and health resources.
  16. Accompany patients to medical appointments, clinic visits or Health Facilities as needed.
  17. Responsible for transporting passengers.
  18. Responsible for clinic vehicle to be in good running condition at all times.
  19. Maintain appropriate, accurate travel records.
  20. Act as clinical staff when needed and within scope of practice.
  21. Develops a quality assurance activity in the CHR program using an internal monitoring system and initiating a corrective action plan.
  22. Other job-related duties as assigned by the Supervisor.

QUALIFICATION REQUIREMENTS

  1. High School Diploma or GED equivalent.
  2. Must have current CPR and First Aid Certification, or willing to be certified within two (2) months.
  3. Must have a valid Nevada driver’s license and be insurable under the tribal vehicle insurance policy.
  4. Prefer a minimum of two (2) years experience in the CHR field.
  5. Must meet training requirements such as driver safety, defensive driving, ADA training and others as required.

  • Ability to maintain accurate client records.
  • Knowledgeable of Privacy Act and able to maintain confidentiality of records.
  • Ability to do vital signs and properly use equipment; to perform assessments and develop SOAP format written Assessment Plan.
  • Must have good communication skills and the ability to motivate patients and interpret for them.
  • Ability to work with Tribal Members on an individual or group basis.
